What is the E-Z App Program?

AFAmeriLife has partnered with EMSI to create the E- Z Application Program. The new EZ-App will be available for use beginning June 1, 2004.

Step 1: Agent Electronic Signature Authorization

To start the process we require that you complete and sign the Agent Electronic Signature
Authorization form large enough to scan in the blank space. This authorizes us to capture an electronic version
of your signature on all future applications resulting from an E-Z form.

Active AFAmerilife agents please fax this form back to us at 817.545.3905. This step is only required once.

Agents submitting activation contracts, please mail this form along with your contract.

DEADLINE: MAY 1, 2004. THIS FORM IS REQUIRED TO REGISTER YOUR AUTHORIZED SIGNATURE. ONLY AGENTS THAT COMPLETE THIS FORM BY THE DEADLINE WILL BE ABLE TO USE THE E-Z APP FORM.

Step 2: The E-Z Form

You must complete an E-Z Form for each applicant. The E-Z form is a lead form, not the final application. Please fill out each E-Z form completely as this helps us do our job more efficiently.

As the agent, you sign and date the first page of the form and the applicant signs and dates the second page. If an Alternative 95 or Ultimate 95 is being applied for, you are required to complete an Illustration Certification. You then fax all pages to the 800# displayed on the application. Once your fax is received, you will receive a return confirmation fax including a contact phone number.

Step 3: What Happens Next?

• The applicant is contacted for a tele-interview.
  • In this tele-interview process the A-1189 application is completed along with any other forms required at the time an application is taken.
  • Your signature is then attached electronically to the completed application.
• EMSI sets up a paramed appointment if required.
  • EMSI sends a packet to the client, which includes the completed application, any required forms, instructions and a postage paid return envelope.
  • The applicant gives the packet to the examiner, or if a non-med application, mails it in themselves.
    Note: If the applicant chooses not to give the packet to the examiner, we will follow up with the applicant daily until received.
• Once we receive all required forms, completed in full, the underwriting progress begins.
• Upon completion of underwriting, the policy is mailed 2-day priority mail to you for delivery.
